Roles & Responsibilities

• Provide skilled physical therapy assistance to residents in a skilled nursing facility, following the supervising PT’s plan of care
• Document treatment sessions and patient progress accurately and timely in accordance with facility and CRG policies
• Collaborate closely with the therapy team and nursing staff to coordinate patient-centered care
• Meet productivity expectations and contribute to efficient, high-quality therapy delivery (facility productivity requirement noted at 92%)
• Adhere to facility policies and procedures, including dress code and attendance expectations

Qualifications

• PTA credential/ certification appropriate for practice (hold current credentials as required)
• 1-3 years of PTA experience (per job posting: 1-3 years)
• Experience or comfort working in a Skilled Nursing Facility (SNF) setting
• Ability to meet productivity standards and follow CRG policies and procedures
• Willingness to follow facility dress code (black scrubs) and be present in the building to receive guaranteed hours

Knowledge & Skills

• Knowledge of SNF therapy practices and resident-centered care
• Strong documentation skills and familiarity with clinical record keeping
• Team-oriented communication and collaboration skills
• Ability to manage time effectively to meet productivity targets
• Comfort with facility policies and regulatory compliance
