Roles & Responsibilities

• Assess patients' physical condition and movement dysfunction through thorough evaluation and history review
• Develop individualized, goal-driven rehabilitation plans to restore function and reduce pain
• Deliver hands-on therapies including manual therapy, massage, and therapeutic exercise instruction
• Coach and educate patients and families on exercises, safety, and strategies for recovery
• Keep detailed, accurate documentation of care plans, progress, and outcomes
• Collaborate with interdisciplinary team members to coordinate the best possible patient care
• Report patient status and progress to providers, family members, and the care committee
• Participate in professional development and in-service training to maintain high standards of care

Qualifications

• Master's Degree from an accredited physical therapy program
• Current Physical Therapist state license
• Current Basic Life Support (BLS) or ability to obtain within 6 months of hire
• Experience working directly with patients in long-term care or hospital settings
• Strong documentation skills and excellent verbal communication
• Proven ability to work as part of a collaborative care team

Education

• Master's Degree in Physical Therapy (accredited program)

• Rehabilitation program planning and individualized treatment design
• Diagnose movement dysfunction and create corrective strategies
• Therapeutic exercise instruction and manual therapy techniques
• Patient coaching, education, and family communication
• Clinical record keeping and accurate documentation
