Roles & Responsibilities

• Provide compassionate, hands-on nursing care for residents in a long-term care setting
• Administer medications and treatments accurately and document care per facility policy
• Collaborate with the interdisciplinary care team to develop and follow individualized care plans
• Perform timely and thorough charting, including MDS-related documentation when applicable
• Monitor resident status and report changes to clinical leadership for prompt intervention
• Support resident dignity and quality of life through person-centered care and communication

Qualifications

• Current, unrestricted Colorado LPN license (required)
• Long-term care (LTC) experience (required)
• MDS (Minimum Data Set) experience (preferred)
• Active clinical skills in medication administration, wound care, and resident assessments

Education

• Graduate of an accredited LPN/LVN program or equivalent practical nursing education

Knowledge & Skills

• Skilled in clinical documentation and charting, including familiarity with MDS processes
• Strong observational skills and ability to prioritize resident needs
• Excellent communication and teamwork within a long-term care environment
• Comfortable with time management for high-quality, efficient resident care
• Ability to follow facility policies, infection control standards, and safety protocols
