Roles & Responsibilities
- Provide compassionate, resident-centered nursing care as a licensed practical nurse
- Administer medications and treatments accurately and document using PCC
- Perform regular resident assessments and report changes to the care team
- Collaborate with RNs, CNAs, therapists, and physicians to deliver coordinated care
- Maintain clear, timely electronic charting in PointClickCare to support continuity
- Assist with admissions, discharges, and transfers while ensuring safety and dignity
- Promote infection control practices and adhere to facility policies and procedures
Qualifications
- Active LPN license in the state of Vermont (required)
- Proficiency in PointClickCare (PCC) is required
- Comfortable floating to different units; flexibility is preferred
- Able to start the assignment on 2026-02-23 and complete through 2026-05-25
- Strong communication skills and a collaborative attitude
Education
- Graduate of an accredited LPN program or equivalent practical nursing education
Knowledge & Skills
- Hands-on clinical skills for long-term and post-acute care
- Accurate medication administration and charting in PCC
- Resident assessment and care planning
- Time management and prioritization during busy shifts
- Teamwork, adaptability, and a positive bedside manner
