Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents at Bear Canyon Rehab Center
• Administer medications and document accurately according to facility policy
• Perform routine assessments and report changes in patient condition to the care team
• Support wound care, mobility assistance, and activities of daily living with dignity
• Work collaboratively with RNs, therapists, and providers to implement care plans
• Maintain clear, timely charting in the electronic medical record
• Participate in shift handoffs and communicate care needs effectively to ensure continuity

Qualifications

• Active LPN license in New Mexico or eligibility to obtain state licensure
• Current BLS/CPR certification
• Previous experience in long-term care, rehabilitation, or skilled nursing preferred
• Strong clinical judgment, documentation skills, and ability to work independently overnight
• Comfortable working as part of an interdisciplinary team and following facility policies

Education

• Diploma or certificate in Practical Nursing (LPN/LVN program)
• High school diploma or equivalent

Knowledge & Skills

• Clinical assessment and basic nursing interventions
• Medication administration and safe documentation practices
• Wound care basics and proper infection control techniques
• Familiarity with electronic medical records and accurate charting
• Effective communication with patients, families, and the care team
• Time management and prioritization during overnight shifts
