Roles & Responsibilities

• Provide compassionate, hands-on nursing care to patients in the Uptown Health/Rehab area
• Administer medications and treatments accurately and document appropriately
• Perform thorough patient assessments and report changes in condition promptly
• Collaborate with interdisciplinary team members to create and implement care plans
• Maintain clear, timely electronic health record documentation and shift reports
• Support patient and family education to promote recovery and safe transitions
• Adhere to infection control, safety protocols, and facility policies to protect patients and staff

Qualifications

• Valid Licensed Practical Nurse (LPN) license in New Mexico (Registered Nurse license considered if applying as RN)
• Current Basic Life Support (BLS) certification preferred or willingness to obtain
• Clinical experience in med-surg, rehab, or long-term care is a plus
• Strong communication skills and a team-first attitude
• Ability to handle a fast-paced clinical environment and prioritize patient needs

Education

• Graduate of an accredited LPN program (or RN program if applying as RN)
• High school diploma or equivalent required

Knowledge & Skills

• Medication administration and documentation best practices
• Patient assessment and basic clinical monitoring
• Wound care fundamentals and dressing management
• Familiarity with electronic health record systems and accurate charting
• Effective patient and family education techniques
• Team collaboration and clear handoff communication
