Roles & Responsibilities
- Assess and monitor resident health status, reporting changes promptly to the care team
- Administer medications and treatments accurately and document thoroughly
- Perform routine nursing procedures and assist with activities of daily living
- Collaborate with RNs, therapists, and support staff to deliver person-centered care
- Maintain clear, timely charting in the resident record and communicate with families
- Advocate for resident comfort, dignity, and safety while fostering a compassionate environment
Qualifications
- Valid LPN license (Licensed Practical Nurse) — New Hampshire licensure preferred/required
- Current BLS/CPR certification (or able to obtain upon hire)
- Experience in long-term care or skilled nursing is a plus
- Ability to pass required background checks and health screenings
- Local candidates only (per facility note)
Education
- Vocational Nursing diploma or equivalent; LPN license required
Knowledge & Skills
- Medication administration and accurate documentation
- Basic wound care and infection prevention practices
- Comfort with activities of daily living (ADL) support and resident assistance
- Familiarity with electronic medical records/charting
- Strong communication, teamwork, and time-management skills
- Clinical judgment and the ability to prioritize resident needs
