Roles & Responsibilities

• Provide compassionate nursing care to residents, focusing on safety and dignity
• Administer medications and treatments accurately and document in the chart
• Monitor patient conditions, report changes to the care team, and escalate concerns
• Collaborate with interdisciplinary teams to develop and implement care plans
• Perform routine assessments, wound care, and assist with activities of daily living
• Educate patients and families about care plans, medications, and follow-up

Qualifications

• Active Licensed Practical Nurse (LPN) license — New Mexico licensure required or transferable
• Basic Life Support (BLS/CPR) certification preferred or willing to obtain
• Up-to-date immunizations and TB screening per facility policy
• Comfort with long-term care or rehabilitation environments is a plus
• Ability to pass background check and drug screen as required

Education

• Completion of an accredited LPN/LVN program
• High school diploma or equivalent

Knowledge & Skills

• Strong medication administration and documentation skills
• Solid understanding of infection control and patient safety practices
• Good clinical assessment skills and ability to recognize changes in condition
• Clear, compassionate communication with patients, families and teammates
• Familiarity with electronic charting and care documentation (EMR experience helpful)
