Roles & Responsibilities

• Conduct compassionate home health visits to assess patient needs and progress
• Develop and implement individualized care plans in partnership with patients and families
• Provide skilled nursing care and hospice support when appropriate
• Educate patients and caregivers on condition management, medications, and safety
• Collaborate with the interdisciplinary team to ensure seamless continuity of care
• Document clinical findings and visit details accurately and timely in the record system
• Recognize changes in patient condition and escalate concerns to the care team

Qualifications

• Registered Nurse with experience or interest in home health and hospice care
• Comfortable performing in-home visits and working independently in community settings
• Strong interpersonal skills to engage patients, families, and the care team
• Ability to document care clearly and follow agency policies for home health visits

Knowledge & Skills

• Skilled at conducting home health assessments and delivering patient-centered care
• Excellent communication and education skills for patients and families
• Good time management and organization to manage a visit-based schedule
• Ability to collaborate with interdisciplinary teams and follow care plans
• Comfortable working in diverse home environments and adapting to patient needs
