Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in a skilled nursing setting
• Administer medications and treatments safely and in accordance with facility policies
• Perform thorough assessments and document clinical findings accurately in the medical record
• Collaborate with interdisciplinary team members to develop and implement individualized care plans
• Monitor residents for changes in condition and escalate concerns promptly to the care team
• Support admissions, discharges, and transitions with clear communication and attention to detail
• Coach and educate residents and families on care needs, safety, and recovery goals

Qualifications

• Active Registered Nurse license in Massachusetts (required)
• Current BLS certification (required)
• Experience in long-term care, rehab, or skilled nursing preferred
• Strong clinical assessment, medication administration, and documentation skills
• Excellent communication and teamwork abilities with a compassionate bedside manner
• Ability to work the evening shift, 3:00 PM - 11:00 PM

Education

• Graduate of an accredited nursing program (ASN/ADN/BSN) for RN roles
• Practical Nursing diploma or certificate for LPN applicants (facility may consider both LPN and RN)

Knowledge & Skills

• Medication administration and charting in electronic health records
• Resident assessment, wound care basics, and care planning
• Familiarity with skilled nursing and rehabilitation workflows
• Ability to prioritize tasks in a dynamic care environment
• Effective collaboration with nursing assistants, therapists, and providers
