Roles & Responsibilities

• Provide compassionate, skilled nursing care to residents, starting each shift with a thorough assessment
• Administer medications and treatments accurately and timely while following facility policies
• Document resident status, care provided, and any changes clearly and professionally in the charting system
• Collaborate with interdisciplinary team members to create and follow individualized care plans
• Monitor and report changes in condition, escalating concerns to nursing leadership promptly
• Support residents with activities of daily living while encouraging independence and dignity

Qualifications

• Current Licensed Practical Nurse (LPN) license — must be valid in New Hampshire
• Graduation from an accredited LPN program or equivalent practical nursing education
• Current BLS/CPR certification or willingness to obtain upon hire
• Comfortable working the Sunday-Thursday, 3:00 PM - 11:30 PM schedule
• Strong communication skills, attention to detail, and a team-oriented mindset
• Previous long-term care or skilled nursing experience is a plus, but enthusiasm and solid fundamentals matter

Education

• Completion of an accredited Practical Nursing (LPN) program

Knowledge & Skills

• Resident assessment and documentation with accuracy and clarity
• Medication administration and IV/shot familiarity appropriate to LPN scope
• Basic wound care and infection prevention practices
• Electronic charting/EHR comfort for timely documentation
• Effective communication with residents, families, and clinical teams
• Ability to prioritize tasks in a busy evening-to-night clinical setting
