Roles & Responsibilities
- Provide compassionate, hands-on care to residents with dignity and respect
- Administer medications and treatments accurately and document in resident records
- Perform thorough assessments and promptly communicate changes to the care team
- Collaborate with nurses, therapists, and support staff to create and follow individualized care plans
- Assist with activities of daily living (ADLs) and encourage resident independence
- Support safe transfers, mobility assistance, and fall-prevention measures
- Participate in shift handoffs and maintain clear, accurate clinical documentation
Qualifications
- Current, active LPN or RN license (Massachusetts) — must be in good standing
- BLS/CPR certification (or willingness to obtain upon hire)
- Previous experience in long-term care, rehabilitation, or skilled nursing is a strong plus
- Excellent communication skills and a collaborative mindset
- Ability to pass required background checks and health screenings
Education
- Graduate of an accredited LPN program or nursing program (LPN/RN appropriate)
- Additional certifications or continuing education in geriatric care or rehab nursing encouraged
Knowledge & Skills
- Solid clinical assessment and basic nursing skills for adult and geriatric populations
- Medication administration knowledge and accurate charting practices
- Comfortable working in a fast-paced rehab/skilled nursing environment
- Strong teamwork, time management, and patient-centered care approach
- Familiarity with infection control and safety procedures
