Roles & Responsibilities

• Provide compassionate, hands-on care to residents with dignity and respect
• Administer medications and treatments accurately and document in resident records
• Perform thorough assessments and promptly communicate changes to the care team
• Collaborate with nurses, therapists, and support staff to create and follow individualized care plans
• Assist with activities of daily living (ADLs) and encourage resident independence
• Support safe transfers, mobility assistance, and fall-prevention measures
• Participate in shift handoffs and maintain clear, accurate clinical documentation

Qualifications

• Current, active LPN or RN license (Massachusetts) — must be in good standing
• BLS/CPR certification (or willingness to obtain upon hire)
• Previous experience in long-term care, rehabilitation, or skilled nursing is a strong plus
• Excellent communication skills and a collaborative mindset
• Ability to pass required background checks and health screenings

Education

• Graduate of an accredited LPN program or nursing program (LPN/RN appropriate)
• Additional certifications or continuing education in geriatric care or rehab nursing encouraged

Knowledge & Skills

• Solid clinical assessment and basic nursing skills for adult and geriatric populations
• Medication administration knowledge and accurate charting practices
• Comfortable working in a fast-paced rehab/skilled nursing environment
• Strong teamwork, time management, and patient-centered care approach
• Familiarity with infection control and safety procedures
