Roles & Responsibilities
- Provide compassionate bedside care and adhere to individualized care plans
- Administer medications and treatments safely and accurately
- Monitor and document patient vital signs, responses, and progress
- Collaborate with RNs, therapists, and the interdisciplinary team for coordinated care
- Support resident mobility, ADLs, and rehabilitation goals with dignity
- Communicate clearly with patients and families to keep them informed and comfortable
- Maintain timely, thorough charting in the facility’s clinical documentation system
Qualifications
- Active LPN (Licensed Practical Nurse) license in good standing
- Minimum clinical experience as an LPN preferred (specific years negotiable)
- Current BLS/CPR certification (or ability to obtain prior to start)
- Reliable transportation required (facility note: must have dependable means to commute)
- Ability to work night shift schedule and every-other-weekend requirement
- Comfortable working in long-term care/rehab settings and following facility protocols
Education
- High school diploma or GED required
- Graduation from an accredited LPN/LVN program required
- LPN licensure in the state of Vermont required
Knowledge & Skills
- Strong medication administration and charting skills
- Skilled in vital sign assessment and early recognition of changes
- Knowledge of infection control and wound care basics
- Good interpersonal and teamwork communication skills
- Time management and prioritization in a fast-paced clinical environment
- Familiarity with electronic medical records and documentation workflows
