Roles & Responsibilities
- Provide compassionate bedside care to residents in a skilled nursing setting
- Administer medications and treatments accurately and safely during your shift
- Perform timely assessments and monitor changes in patient condition
- Document care clearly and thoroughly in the resident chart
- Collaborate with nurses, therapists, and the care team to create great care plans
- Support residents with activities of daily living and promote dignity and independence
- Communicate with families and providers with clarity and empathy
Qualifications
- Active LPN license (or RN if applying to the RN opening) — appropriate credentials for the role
- Comfort and ability to work a 12-hour day shift (8:00 AM - 8:00 PM)
- Reliable, compassionate approach to resident-centered care
- Ability to perform nursing duties in a long-term/rehab setting
- Willingness to complete facility-required onboarding and immunizations
Education
- Completion of an accredited LPN program (or ADN/BSN for RN applicants as applicable)
- High school diploma or equivalent
Knowledge & Skills
- Strong clinical assessment and medication administration skills
- Infection control and safe patient handling practices
- Clear documentation and charting abilities
- Excellent communication and teamwork skills
- Time management and the ability to prioritize care tasks
