Roles & Responsibilities

• Provide skilled physical therapy evaluations and evidence-based treatment plans for residents
• Deliver therapeutic interventions—strengthening, balance, gait training, and functional mobility work
• Monitor progress and modify plans of care to help patients reach meaningful goals
• Collaborate with nursing and rehab teams to coordinate safe, person-centered care
• Document thoroughly and timely to meet facility and regulatory requirements
• Educate patients, families, and caregivers to support carryover and safe transitions

Qualifications

• Licensed Physical Therapist in Texas or eligible to obtain Texas licensure
• Degree in Physical Therapy (DPT, MPT, or equivalent) preferred
• BLS/CPR certification required (or able to obtain prior to start)
• Experience in skilled nursing facility or rehabilitation settings is highly desirable
• Strong communication skills and a collaborative approach to interdisciplinary care
• Ability to complete a 13-week travel assignment and work Monday–Friday schedule

Education

• DPT preferred; Bachelor or Master in Physical Therapy considered

Knowledge & Skills

• Skilled in performing comprehensive PT evaluations and developing individualized treatment plans
• Familiar with therapeutic exercise, manual techniques, gait training, and ADL-focused interventions
• Knowledgeable in safe patient handling and fall prevention strategies
• Comfortable with electronic documentation and meeting regulatory documentation standards
• Ability to teach and motivate patients and family members for better outcomes
