Roles & Responsibilities

• Assess resident functional status and rehabilitation needs with care and accuracy
• Develop individualized treatment plans that promote independence and safety
• Deliver hands-on therapy and therapeutic exercise with empathy and skill
• Collaborate with nursing and interdisciplinary teams to coordinate care
• Document progress clearly and timely to support outcomes and continuity
• Educate residents and families on home programs and fall prevention strategies
• Adapt interventions to the SNF population, focusing on functional goals

Qualifications

• Licensed PT or DPT (new graduates welcome — any experience level considered)
• Comfortable working in a Skilled Nursing Facility (SNF) setting
• Strong communication skills and a team-first attitude
• Able to complete documentation in facility charting systems
• Active professional liability insurance or ability to obtain as required

Education

• Doctor of Physical Therapy (DPT) preferred — new grads accepted

Knowledge & Skills

• Skilled assessment of mobility, balance, transfers, and functional mobility
• Experience or comfort with restorative nursing and long-term care goals
• Knowledge of therapeutic exercise, gait training, and assistive devices
• Ability to write clear, goal-oriented treatment plans and progress notes
• Collaborative approach to care planning with interdisciplinary teams
