Roles & Responsibilities

• Provide skilled physical therapy assisting interventions to residents in a skilled nursing setting
• Collaborate closely with the interdisciplinary care team to create and follow individualized care plans
• Document patient progress accurately and timely in the medical record
• Educate residents and their families on safe mobility, therapeutic exercises, and fall prevention strategies
• Support safe transfers, ambulation, and use of adaptive equipment
• Participate in team meetings, care planning, and discharge planning to ensure continuity of care

Qualifications

• Licensed PTA in Rhode Island or eligible to obtain Rhode Island PTA license
• Graduate of an accredited Physical Therapist Assistant program (new grads encouraged to apply)
• Current BLS/CPR certification or willingness to obtain prior to start
• Comfortable working in a SNF environment with strong clinical judgment
• Reliable, professional, and able to pass required background checks and screenings
• Any experience level accepted, including New Grad

Education

• Associate Degree in Physical Therapist Assistant (or equivalent). New graduates welcome.

Knowledge & Skills

• Solid knowledge of rehabilitation techniques appropriate for geriatric and SNF populations
• Proficient in patient assessment, therapeutic exercise, gait training, and ADL support
• Strong communication and documentation skills
• Ability to work collaboratively within an interdisciplinary team
• Comfortable with the physical demands of the role and safe patient handling principles
• Familiarity with electronic medical record systems preferred
