Roles & Responsibilities

• Provide compassionate, safe nursing care to skilled nursing facility residents
• Administer medications and treatments accurately and document thoroughly
• Perform focused assessments and communicate changes in condition to the care team
• Assist with activities of daily living while promoting resident dignity and independence
• Collaborate with nurses, therapists, and other staff to deliver person-centered care
• Support infection control and safety practices in accordance with facility policies

Qualifications

• Valid LPN license in good standing (current or compact where applicable)
• Experience or comfort working in a skilled nursing facility preferred
• Up-to-date immunizations and health documentation as required: MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza
• Current physical/health statement and ability to pass Trinity 10 panel drug screen
• Strong communication skills and the ability to work well in a team
• Reliable, professional, and patient-focused approach to care

Education

• Graduate of an accredited practical nursing program and active LPN license

Knowledge & Skills

• Medication administration and basic IV/shot experience as applicable
• Resident assessment, documentation, and care planning
• Basic wound care and catheters/ostomy care familiarity
• Infection prevention and control practices
• Time management and prioritization in a fast-paced SNF environment
• Empathy, patience, and strong interpersonal skills
