Roles & Responsibilities

• Provide high-quality outpatient physical therapy care, including evaluations, treatment planning, and interventions
• Develop and update individualized treatment plans to help patients meet functional goals
• Document patient care thoroughly and maintain current license and certification records
• Complete the required Physical Therapy skills checklist and submit required documentation
• Coordinate with NCH credentialing and complete provider enrollment prior to seeing patients
• Collaborate with clinic staff and multidisciplinary teams to ensure continuity of care
• Be flexible with scheduling to support clinic needs, including late-day shifts when needed

Qualifications

• Minimum 2 years current outpatient physical therapy experience (required)
• Active New Hampshire PT license (compact allowed) and ability to provide license verification
• Current American Heart Association BLS certification (required)
• Physical Therapy skills checklist completed and submitted (required)
• One supervisory-signed reference from the past year of employment (required)
• Willingness to complete provider enrollment prior to patient care (enrollment may take 60+ days)
• Permanent address included in submission materials and RTO information provided
• Previous travel experience highly preferred; flexible scheduling mindset

Knowledge & Skills

• Outpatient physical therapy assessment and treatment skills
• Understanding of provider enrollment and insurance credentialing processes
• License verification and documentation management
• American Heart Association BLS competency
• Ability to work independently in an outpatient clinic and collaborate with clinic staff
• Comfort adapting schedule to meet clinic needs, including late days
