Shift Type: Day shifts, 8-hour shifts (shift length: 8 hours), every-other-weekend rotation (EOW), First-to-float (will float to all units excluding surgery); guaranteed 40 hours/week
Hours per Week: 40
Candidate Pay Rate: $82 per hour

Roles & Responsibilities
- Coordinate and manage patient care across the hospital as the Care Coordinator, ensuring smooth transitions and clear communication
- Conduct daily case management activities and manage a caseload (approximately 15–25 patients per day, may vary with staffing)
- Collaborate with interdisciplinary teams to plan and execute discharges, transfers, and post-acute care arrangements
- Document thoroughly and accurately in Cerner to maintain continuity of care and compliance
- Provide patient and family education, support care planning, and advocate for patient needs
- Float to assigned units (excluding surgery) as needed and adapt to a variety of patient populations including cardiac, surgical, oncology, and critical care
- Complete a one-day orientation with an RNCC and maintain required certifications and documentation
Qualifications
- Active California Registered Nurse (RN) license required
- Minimum 2 years of nursing experience (as listed)
- Recent experience as a Care Coordinator at a California Dignity Health hospital is required by the Director (candidates without this specific experience will not be considered)
- BLS certification required
- Familiarity with Cerner charting system
- Two supervisor references required at submittal (charge accepted): one within 12 months and one within 3 years
- Candidates must not have been directly employed by any CommonSpirit/Dignity facility within the past year (PRN employment through outside firms is acceptable)
Education
- Active California RN license (required)
- BSN not required
Knowledge & Skills
- Care coordination and case management in an acute care setting
- Strong clinical assessment and prioritization skills
- Experience with cardiac, surgical, oncology and critical care patient populations
- Proficient documentation skills in Cerner
- Ability to float across multiple units and adapt quickly
- Excellent communication and interdisciplinary collaboration abilities
