Roles & Responsibilities

• Perform Start of Care (SOC) visits and ongoing PT visits in the home setting
• Provide orthopedic and joint replacement rehabilitation tailored to each patient
• Manage an expected caseload of approximately 5 visits per day
• Document thoroughly and timely in HCHB (HomeCare HomeBase) and Cerner when required
• Collaborate with the Intake RN, manager, nurse aides, and other therapy disciplines
• Assess home safety, mobility needs, and create individualized treatment plans
• Educate patients and caregivers on exercises, fall prevention, and recovery milestones

Qualifications

• Valid Physical Therapist license (California) — license to submit is required
• Home Health experience required; candidates without home health experience will not be considered
• BLS (AHA) certification required
• 1 year of PT experience preferred; will consider travelers with less than 2 years if they have Home Health experience
• Comfortable using tablet-based EMR (HCHB) and able to adapt to Cerner charting as needed
• Reliable transportation and ability to travel within Tehama County for patient visits

Knowledge & Skills

• Proficiency with HomeCare HomeBase (HCHB) EMR charting
• Familiarity with Cerner is a plus
• Expertise in orthopedic rehabilitation and joint replacement recovery
• Experience caring for geriatric patients with comorbidities such as CHF and COPD
• Strong time management to handle multiple home visits per day
• Excellent patient/caregiver education and communication skills
• Ability to conduct home assessments and practical hands-on mobility training
