Roles & Responsibilities

• Provide skilled home health nursing including start of care, recertifications, resumptions of care, and routine visits to prevent rehospitalization
• Perform wound care and manage wound VACs with clinical precision
• Manage IV/PICC line care and monitor for complications
• Assess and manage patients with CHF and COPD to optimize outcomes and reduce readmissions
• Deliver clear, compassionate patient and caregiver education, including ventilator/CPAP/BiPAP basics when applicable
• Chart thoroughly and timely in Cerner to maintain excellent clinical documentation
• Collaborate with home health aides and the interdisciplinary team to coordinate patient-centered care
• Communicate effectively with leadership for routine questions and escalate urgent needs to on-call administration

Qualifications

• Active RN license (California) required
• BLS certification required
• Valid driver’s license and vehicle insurance required for home visits
• Minimum 2 years of RN experience (as listed in job details)
• Home Health experience and comfort with high-acuity, recently discharged patients preferred
• Experience with wound care, IV/PICC line management, CHF and COPD care preferred
• Familiarity with Cerner charting; HCHB (HomeCare HomeBase) experience is a plus
• Reliable transportation and ability to complete ~5 visits per day (point-based visit system)

Education

• BSN not required (ADN or nursing diploma accepted)
• Active California RN license required

Knowledge & Skills

• Proficient charting in Cerner and comfort with electronic health records
• Knowledge of home health workflows including start of care and recertification processes
• Skilled in wound assessment and advanced wound care techniques
• Ability to educate patients and families on respiratory equipment (vent, CPAP, BiPAP) at a functional level
• Strong time management, prioritization, and clinical decision-making for community-based care
• Excellent communication and teamwork to coordinate with home health aides and on-call leadership
