Roles & Responsibilities

• Provide skilled physical therapy evaluations and individualized treatment plans in patients' homes
• Perform thorough assessments and document findings accurately using EPIC/OASIS
• Develop and teach effective Home Exercise Programs that empower patients and families
• Collaborate with nursing and interdisciplinary team members to coordinate safe, patient-centered care
• Manage your caseload efficiently, typically up to six patient visits per day while maintaining quality
• Educate patients and caregivers on body mechanics, fall prevention, and post-operative precautions
• Maintain reliable transportation and minimize drive time while prioritizing patient safety

Qualifications

• Current Physical Therapist license or eligible to practice in Oregon (pending license accepted)
• Minimum 1 year of skilled home health experience required
• BCLS certification required
• Experience with EPIC and OASIS charting required; please call out if you lack OASIS experience
• Ability to safely drive between patient visits and work independently in home settings
• Primary COVID-19 vaccination series plus one booster required by facility
• Strong communication skills for patient and family education

Knowledge & Skills

• Skilled in patient assessment and developing Home Exercise Programs
• Solid understanding of body mechanics and fall prevention
• Experience with orthopedics: post-op shoulder, spine, total hip and knee replacements
• Neurologic rehab experience: post-stroke, TBI, spinal cord injury, MS, Parkinson’s
• Comfortable charting in EPIC and completing OASIS documentation
• Patient and family education, coaching, and motivational techniques
• Ability to prioritize and manage up to 6 visits per day while meeting productivity expectations
