Roles & Responsibilities

• Perform thorough patient assessments during home visits to develop tailored care plans
• Deliver skilled physical therapy interventions for orthopedic and neurologic conditions
• Create and teach effective home exercise programs to improve patient outcomes
• Document all visits accurately and promptly in EPIC and OASIS
• Educate patients and families to promote independence and safety at home
• Manage a typical caseload of up to six visits per day while maintaining quality
• Collaborate with interdisciplinary team members and coordinate referrals as needed
• Maintain professional standards, complete required certifications, and follow facility vaccine guidelines

Qualifications

• Current or pending Physical Therapy license (pending license accepted; confirm state requirements)
• Minimum 1 year of skilled home health physical therapy experience required
• BCLS certification required
• Experience with EPIC charting and OASIS is required; please note if you lack OASIS experience
• Proof of primary COVID-19 vaccination series plus booster required by the facility
• Reliable transportation for home visits and ability to travel occasional longer-radius routes

Knowledge & Skills

• Physical Therapy best practices for home health settings
• Patient assessment and functional outcome measurement
• EPIC and OASIS charting workflows
• Designing Home Exercise Programs (HEP)
• Patient and family education techniques
• Orthopedics: total hip/knee replacement, post-op care, shoulder and spinal surgery rehab
• Neurologic conditions: stroke rehab, Parkinson's disease, MS, TBI, spinal cord injury
• Body mechanics and safe patient handling
• Time management to handle up to six visits per day
• Ability to work across settings including outpatient, acute care, SNF, and rehabilitation hospital contexts
