Shift Type: Primarily Monday-Friday, 8:00 AM - 4:30 PM (Day shift); 5 days/week with every-other-weekend rotation (every other weekend). Includes a 30-minute lunch (clock out) and two 15-minute paid breaks.

Candidate Pay Rate: $82 per hour

Roles & Responsibilities

• Conduct utilization review for all inpatients using InterQual criteria and Cerner documentation—ensure appropriate level of care decisions.
• Lead and oversee discharge planning activities to ensure safe, timely transitions from the hospital.
• Coordinate patient care with interdisciplinary teams, including physicians, social workers and case managers, to meet clinical and discharge goals.
• Document clinical findings, utilization decisions and discharge plans accurately in the EMR (Cerner).
• Educate patients and families on discharge plans and available resources to support successful transitions.
• Participate in quality improvement initiatives related to utilization management and care coordination.

Qualifications

• Active nursing license required (License to submit: Yes).
• BLS certification required.
• Minimum 3 years of acute care hospital experience with hands-on case management (per job requirements).
• Proven experience in utilization review and discharge planning; ability to perform UR and oversee DC planning.
• Experience with Cerner and InterQual (EMR and utilization review tools) required.
• Previous travel nurse experience highly desirable but not mandatory.
• Comfort working inpatient acute care in a case management/care coordination unit.
