Roles & Responsibilities

• Provide compassionate, patient-centered personal care during home and facility visits
• Assist patients with activities of daily living to promote comfort and dignity
• Support pain and symptom management plans under clinical direction
• Document care thoroughly and communicate changes in condition to the care team
• Collaborate with nurses, social workers, volunteers and family members to deliver holistic hospice care
• Offer bereavement support and compassionate presence to patients and families
• Maintain reliable transportation and ensure timely travel between patient locations

Qualifications

• Current, unencumbered Oregon CNA certification required upon submission
• Healthcare provider-level BLS required
• Valid driver’s license and current auto liability/property damage insurance required
• One year of hospital or long-term care experience preferred
• Home health, hospice or community service experience preferred
• Ability to work independently in community and home settings and collaborate with the interdisciplinary team

Knowledge & Skills

• Solid understanding of hospice philosophy and end-of-life care principles
• Experience with home visits and community-based patient care
• Skilled in personal care, infection control and safe patient handling
• Excellent documentation, communication and interpersonal skills
• Empathy, patience and cultural sensitivity when supporting patients and families
• Time management and reliable transportation for traveling between patient sites
