Roles & Responsibilities
- Screen and evaluate patients with cognitive and/or ADL physical functioning disorders to determine needs and goals
- Develop and implement individualized plans of care that follow regulatory and clinical practice standards
- Use standardized assessments and evidence-based practice to guide interventions and measure progress
- Provide rehabilitative and medically necessary skilled treatments, including ADL retraining, therapeutic activities, and therapeutic exercises
- Administer modalities and recommend/obtain supportive and assistive devices (DME) as needed
- Assess patient progress, document outcomes, and modify services based on evaluation results
- Prepare comprehensive discharge summaries and communicate plans with patients, families, and interdisciplinary team members
- Accurately document treatment minutes and maintain patient records in the electronic documentation system
- Participate in patient care conferences, utilization review, rehabilitation meetings, and required trainings
- Supervise and support occupational therapy assistants, rehabilitation aides, and students when applicable
Qualifications
- Active Occupational Therapy license required (Massachusetts state licensure)
- Minimum 1 year of OT clinical experience (required)
- Experience in Long-Term Care (LTC) or rehabilitation settings (required)
- Familiarity with NBCOT/AOTA standards and adherence to state practice acts and ethical codes
- Proven ability to perform standardized assessments and develop effective plans of care
- Comfortable supervising OT assistants and working collaboratively with interdisciplinary teams
Knowledge & Skills
- Skilled in patient evaluation and plan of care development
- Proficient with standardized assessments and evidence-based practice
- Experience with ADL retraining, therapeutic activities, and therapeutic exercises
- Knowledge of modalities and supportive/assistive device (DME) procurement
- Strong documentation skills in electronic documentation systems and accurate treatment minute recording
- Ability to counsel and educate patients, families, and caregivers
- Good communication and collaboration with supervisors and health team members
- Ability to prepare discharge summaries and participate in utilization and rehabilitation reviews
