Roles & Responsibilities

• Assess patients' mobility, strength, balance and functional needs to create a baseline for care
• Develop individualized treatment plans that target meaningful, measurable goals
• Deliver evidence-based therapeutic interventions including exercise, manual therapy and patient education
• Document patient progress clearly and timely in the medical record to support continuity of care
• Collaborate with multidisciplinary teams to coordinate discharge planning and maximize outcomes
• Educate patients and families on home exercise programs and safety strategies to support independence
• Monitor and adjust treatment plans based on progress and clinical judgment
• Maintain a safe, professional environment and adhere to facility and regulatory guidelines

Qualifications

• Valid Physical Therapist license in California or ability to obtain licensure prior to start
• Current Basic Life Support (BLS/CPR) certification (or commitment to obtain upon hire)
• Demonstrated clinical experience in rehabilitation settings; travel therapy experience a plus
• Strong documentation skills and comfort using electronic medical records
• Excellent communication and teamwork skills with a patient-centered approach

Education

• Doctor of Physical Therapy (DPT) preferred, or equivalent degree in Physical Therapy

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise prescription and manual techniques
• Knowledgeable about rehab protocols for orthopedic and medically complex patients
• Proficient with outcome measures, therapy documentation and care planning
• Ability to educate patients and family members in clear, compassionate ways
• Time management and prioritization skills to manage a full caseload effectively
