Roles & Responsibilities

• Provide compassionate direct patient care and assist with activities of daily living
• Monitor and report changes in patient condition to the nursing team
• Take and record vital signs accurately and timely
• Assist with safe transfers, ambulation and positioning
• Maintain a clean, safe and comfortable environment for residents
• Support documentation and charting to ensure continuity of care
• Collaborate closely with nurses, therapists and support staff to deliver person-centered care

Qualifications

• Active Certified Nursing Assistant (CNA) certification — required
• CPR/BLS certification preferred or obtained upon hire
• Proof of current immunizations and clear TB screening as required by facility
• Ability to pass background check and drug screen
• Comfortable working evenings and a schedule that includes most weekends
• Strong communication skills and a caring, team-oriented attitude

Education

• High school diploma or GED preferred
• CNA certification (required)

Knowledge & Skills

• Hands-on experience with patient hygiene, feeding and mobility assistance
• Accurate vital signs measurement and basic clinical observation
• Familiarity with infection control and safety procedures
• Good documentation and time-management skills
• Empathy, patience and strong interpersonal skills when working with residents and families
• Ability to follow care plans and take direction from nursing staff
