Roles & Responsibilities

• Provide compassionate, skilled nursing care to residents and patients during the evening shift
• Administer medications and treatments accurately and document in the patient chart
• Perform and document patient assessments, reporting changes to the RN or provider
• Collaborate with the care team to create and follow individualized care plans
• Assist with wound care, IVs as appropriate to scope, and routine nursing procedures
• Support activities of daily living and ensure patient comfort and safety
• Communicate clearly with families and interdisciplinary team members to coordinate care
• Maintain accurate, timely electronic health record documentation and shift handoffs

Qualifications

• Current LPN license in New Hampshire (required)
• Valid BLS/CPR certification (required)
• Current immunizations and ability to pass background check and drug screen
• Previous experience in long-term care, medical-surgical, or related clinical setting preferred
• Strong patient assessment, medication administration, and documentation skills
• Professional, dependable, and a team player who enjoys hands-on patient care

Education

• Graduate of an accredited Practical Nursing program; LPN/LVN diploma or certificate

Knowledge & Skills

• Knowledge of medication administration and basic clinical procedures
• Comfortable with electronic health records (EHR) and accurate charting
• Strong clinical assessment and prioritization skills
• Excellent communication and teamwork abilities
• Ability to provide compassionate, patient-centered care in a fast-paced setting
• Basic wound care, infection control, and safety protocol knowledge
