Roles & Responsibilities

Provide compassionate nursing care to clinic patients within the scope of LPN practice, making every patient encounter count
Take and interpret vital signs and perform patient assessments to support timely provider care
Triage patient calls efficiently, prioritizing urgent needs and coordinating follow-up
Manage prescription refills and complete prior authorizations to keep patients on their meds
Serve as the primary liaison between patients, clinical staff, and providers to ensure smooth care delivery
Collaborate with the interdisciplinary team and give direction to CNAs and schedulers when appropriate
Follow state and federal licensure and continuing education requirements to maintain clinical competence

Qualifications

Current State LPN license (required)
BLS (Basic Life Support) certification required
Graduate of an accredited Nursing Program (LPN)
Experience taking vital signs, triaging patient calls, and managing prescription refills/prior authorizations
Strong patient communication and teamwork skills
Comfort supervising CNAs and coordinating with providers, clinical managers, and RNs

Education

Graduate of a Nursing Program (LPN)

Knowledge & Skills

Proficient in taking and interpreting vital signs
Triage skills for clinic-based patient assessment
Knowledge of prescription refill workflows and prior authorization processes
Excellent patient communication and customer service
Ability to collaborate with interdisciplinary teams and follow clinical protocols
Familiarity with basic clinical documentation and care coordination
