Roles & Responsibilities

• Provide compassionate nursing care to residents, following individualized care plans
• Administer medications and treatments accurately and document per facility policy
• Monitor resident condition, report changes promptly, and escalate when needed
• Assist with activities of daily living while promoting resident independence
• Collaborate with interdisciplinary team members to deliver coordinated care
• Maintain accurate and timely documentation in resident charts and electronic records
• Support infection control and safety protocols to protect residents and staff

Qualifications

• Current LPN license in Pennsylvania (or compact/state-appropriate) required
• Completion of an accredited practical/vocational nursing program
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Prior experience in long-term care, skilled nursing, or related setting preferred
• Up-to-date immunizations and ability to pass background checks
• Strong communication skills and a resident-centered approach to care

Education

• Completion of an accredited LPN/LVN program
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and restorative nursing techniques
• Resident assessment, vital signs monitoring, and clinical observation
• Familiarity with electronic medical records and charting
• Infection control, safety procedures, and regulatory compliance
• Team collaboration and effective communication with residents and families
