Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents, focusing on safety and dignity
• Administer medications and treatments accurately and document all care promptly
• Monitor and record vital signs and changes in patient condition, escalating concerns as needed
• Collaborate with RNs, CNAs, and interdisciplinary teams to create and follow individualized care plans
• Assist residents with activities of daily living while promoting independence and comfort
• Maintain clear, timely charting in the facility record system and support regulatory compliance
• Support admissions, transfers, and discharge processes to ensure smooth transitions of care

Qualifications

• Active LPN license in Pennsylvania (required)
• Completion of an accredited practical nursing program
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Comfortable administering medications and performing basic nursing procedures
• Strong communication skills and the ability to work well within a multidisciplinary team
• Previous experience in long-term care or skilled nursing preferred but not required

Education

• Graduate of an accredited Practical Nursing program (LPN)
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and accurate documentation practices
• Basic nursing assessments and vital sign interpretation
• Familiarity with infection control and patient safety protocols
• Comfort using electronic medical records and completing timely charting
• Compassionate patient interaction and effective family communication
• Time management and the ability to prioritize tasks in a busy care setting
