Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately and on time
Assess patient conditions and document changes clearly in the chart
Collaborate with nurses, therapists, and interdisciplinary team members to deliver coordinated care
Educate residents and families about care plans and health maintenance
Maintain safety, infection control, and a clean care environment
Participate in shift handoffs and contribute to continuous quality improvement

Qualifications

Current LPN license in Pennsylvania (required)
Graduate of an accredited LPN program or equivalent training
Current BLS/CPR certification (or willingness to obtain)
Experience in long-term care, skilled nursing, or related settings preferred
Strong verbal and written communication skills and a team-first attitude
Ability to perform hands-on care, medication administration, and documentation

Education

Completion of an accredited LPN program; High school diploma or GED

Knowledge & Skills

Patient assessment and vital signs monitoring
Medication administration and accurate documentation
Care planning and implementation for activities of daily living (ADLs)
Basic wound care and safe patient handling
Familiarity with electronic health records (EHR) and charting best practices
Infection control, safety protocols, and regulatory compliance
