Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, direct patient care in accordance with care plans
Administer medications and treatments safely and accurately
Monitor patient status and report changes to the care team promptly
Document assessments, interventions and patient responses in the chart
Support residents’ daily living needs with dignity and professionalism
Collaborate with nurses, therapists and support staff to deliver coordinated care

Qualifications

Active LPN license in Pennsylvania (required)
Graduation from an accredited practical nursing program
Current BLS/CPR certification (or willingness to obtain upon hire)
Up-to-date immunizations and ability to pass background and drug screening
Clinical experience in long-term care, skilled nursing or similar setting preferred

Education

Completion of an accredited Practical Nursing program and LPN/LVN diploma or certificate

Knowledge & Skills

Strong medication administration and wound care skills
Proficient with nursing documentation and basic EHR navigation
Knowledge of infection prevention and safety protocols
Excellent communication and teamwork skills with patients and families
Ability to prioritize tasks in a fast-paced care environment
