Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
Administer medications and treatments accurately and document thoroughly
Perform timely assessments and communicate changes in patient condition to the care team
Collaborate with interdisciplinary staff to develop and follow individualized care plans
Support residents with activities of daily living while promoting independence and dignity
Maintain accurate, timely charting in the resident record and follow facility policies

Qualifications

Current LPN license (Practical Nurse) — ability to obtain/hold Pennsylvania licensure preferred
Current Basic Life Support (BLS) / CPR certification (or willingness to obtain)
Comfortable administering meds, charting, and working in a skilled nursing environment
Strong communication skills and a team-first attitude
Previous experience in long-term care or skilled nursing is a plus

Education

High school diploma or GED required
Completion of an accredited LPN/LVN program

Knowledge & Skills

Medication administration and documentation best practices
Resident assessment and basic clinical monitoring
Familiarity with care planning and interdisciplinary communication
Infection control and patient safety principles
Basic electronic health record (EHR) navigation and charting
