Roles & Responsibilities

Provide compassionate direct patient care within scope of practice as an LPN
Administer medications and treatments accurately and document accordingly
Monitor patient status and report changes to RN or provider promptly
Maintain clear, timely documentation in the medical record
Collaborate with the interdisciplinary team to support individualized care plans
Ensure patient safety by following infection control and facility protocols
Assist with patient education and family communication in a friendly, clear manner

Qualifications

Active LPN license (eligible to practice in Pennsylvania preferred)
Current BLS/CPR certification or willingness to obtain upon hire
Ability to complete required health screenings and background checks
Comfort working 8-hour shifts and performing standard nursing duties
Strong communication skills and a patient-centered attitude

Education

LPN diploma, certificate, or equivalent practical nursing program completion

Knowledge & Skills

Medication administration and basic clinical assessments
Patient hygiene, wound care support, and ADL assistance
Clinical documentation and charting best practices
Team communication and ability to follow care plans
Basic infection control and safety protocols
