Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents as an LPN
Administer medications and treatments safely and accurately
Perform daily assessments and document patient status in the medical record
Collaborate with RNs, CNAs, and interdisciplinary team members to create and follow care plans
Educate residents and families about care plans and changes in condition
Observe, report, and escalate changes in resident condition promptly
Support a positive resident experience through respectful communication and attention to dignity

Qualifications

Active LPN license in Pennsylvania (or ability to obtain prior to start)
Current BLS/CPR certification
Clinical experience in long-term care, skilled nursing, or similar settings preferred
Strong communication skills and a team-oriented attitude
Ability to perform duties required for an 8.5-hour clinical shift

Education

LPN diploma or vocational nursing certificate
BLS/CPR certification

Knowledge & Skills

Medication administration and documentation
Basic assessment skills and vital signs monitoring
Experience with electronic medical records and charting
Knowledge of infection control practices and safety protocols
Ability to prioritize and multitask in a fast-paced care environment
