Roles & Responsibilities

Provide compassionate, patient-centered nursing care with attention to detail
Administer medications and treatments accurately and safely
Assess and monitor patient condition, reporting changes promptly
Document patient care and observations clearly in the health record
Collaborate with nurses and interdisciplinary team members to deliver coordinated care
Assist with admissions, discharges and patient transfers to ensure smooth transitions
Support infection control and safety protocols to protect patients and staff

Qualifications

Valid LPN license in Pennsylvania (or eligible for PA licensure) — state licensure is required
Current BLS/CPR certification or willingness to obtain upon hire
Clinical experience in a long-term care or skilled nursing setting is a plus
Strong communication skills and a team-first attitude
Ability to perform essential nursing duties safely and reliably

Education

Completion of an accredited LPN/Vocational Nursing program
High school diploma or GED

Knowledge & Skills

Medication administration and documentation best practices
Basic patient assessment and vital signs monitoring
Familiarity with electronic health records and charts
Infection control principles and safe patient handling
Time management and prioritization in a fast-paced clinical setting
Compassionate patient interaction and family communication
