Roles & Responsibilities

• Provide high-quality bedside care and compassionate support to long term care residents
• Administer medications and treatments accurately and document per facility policy
• Monitor resident condition, identify changes, and communicate concerns to the team
• Support residents with activities of daily living while promoting dignity and independence
• Collaborate with nurses, therapists, and care staff to deliver coordinated care plans
• Maintain accurate charts and electronic health records in a timely manner
• Follow infection control, safety standards, and long term care regulations

Qualifications

• Valid LPN license (current or the ability to obtain) is required
• Experience in long term care preferred; demonstrated resident-focused care
• Current BLS/CPR certification or willingness to obtain
• Ability to pass background checks and required health screenings
• Strong communication skills and ability to work well in a team

Education

• Practical nursing diploma or certificate (LPN program) and required state licensure

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and ADL assistance
• Familiarity with long term care policies and resident-centered care
• Comfortable using electronic charting / EHR systems
• Strong time management, observation, and interpersonal skills
