Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day)
Hours per Week: 35
Candidate Pay Rate: $40 per hour

Roles & Responsibilities
- Provide high-quality physical therapy assessments and individualized treatment plans for skilled nursing residents
- Deliver therapeutic interventions including mobility training, gait and balance work, and functional strengthening
- Monitor and document patient progress clearly and timely in the health record
- Collaborate with nurses, occupational therapists, speech therapists and social work to coordinate comprehensive care
- Educate patients and families on home programs, safety, and adaptive strategies to maximize independence
- Participate in care conferences and contribute to improvement initiatives that enhance patient outcomes
Qualifications
- Licensed Physical Therapist (PT) - state licensure required (Wisconsin license expected prior to start)
- Current BLS/CPR certification or willingness to obtain
- Experience or comfort working in skilled nursing or post-acute settings preferred
- Strong clinical reasoning, documentation habits, and teamwork skills
- Outstanding communication skills with patients, families, and the care team
Education
- Degree in Physical Therapy (Bachelor's, Master's or Doctorate in Physical Therapy commonly accepted)
Knowledge & Skills
- Skilled in conducting evaluations and developing individualized PT plans of care
- Proficiency with therapeutic modalities, manual techniques, gait training and transfer training
- Familiarity with rehab documentation and electronic health record systems
- Ability to prioritize caseloads and manage time efficiently
- Compassionate patient education and family engagement skills
