Roles & Responsibilities

• Provide high-quality physical therapy assessments and individualized treatment plans for skilled nursing residents
• Deliver therapeutic interventions including mobility training, gait and balance work, and functional strengthening
• Monitor and document patient progress clearly and timely in the health record
• Collaborate with nurses, occupational therapists, speech therapists and social work to coordinate comprehensive care
• Educate patients and families on home programs, safety, and adaptive strategies to maximize independence
• Participate in care conferences and contribute to improvement initiatives that enhance patient outcomes

Qualifications

• Licensed Physical Therapist (PT) - state licensure required (Wisconsin license expected prior to start)
• Current BLS/CPR certification or willingness to obtain
• Experience or comfort working in skilled nursing or post-acute settings preferred
• Strong clinical reasoning, documentation habits, and teamwork skills
• Outstanding communication skills with patients, families, and the care team

Education

• Degree in Physical Therapy (Bachelor's, Master's or Doctorate in Physical Therapy commonly accepted)

Knowledge & Skills

• Skilled in conducting evaluations and developing individualized PT plans of care
• Proficiency with therapeutic modalities, manual techniques, gait training and transfer training
• Familiarity with rehab documentation and electronic health record systems
• Ability to prioritize caseloads and manage time efficiently
• Compassionate patient education and family engagement skills
