Roles & Responsibilities

• Provide compassionate, patient-centered nursing care in the Uptown Health/Rehab area
• Assess patients, monitor vital signs, and respond to changes in condition
• Administer medications and treatments accurately and safely
• Document care clearly and promptly in the electronic medical record
• Collaborate with nurses, therapists, and other clinicians to coordinate care
• Educate patients and families on care plans, medications, and discharge instructions
• Support rehabilitation goals and assist with safe mobility and ADLs
• Maintain compliance with facility policies, infection control, and safety procedures

Qualifications

• Current LPN or RN license (Licensed Practical Nurse or Registered Nurse) in New Mexico or eligible for licensure
• Basic Life Support (BLS) certification required; additional certifications a plus
• Prior experience in rehab, acute care, or related nursing area preferred
• Strong clinical assessment, medication administration, and charting skills
• Excellent communication and teamwork abilities
• Comfortable working 12-hour style day shifts when needed

Education

• Graduate of an accredited Practical Nursing program for LPNs
• Graduate of an accredited ADN/BSN nursing program for RNs
• State licensure required for the level of practice

Knowledge & Skills

• Patient assessment and documentation best practices
• Medication administration and safety protocols
• Basic wound care and infection control procedures
• Familiarity with electronic medical records and timely charting
• Rehab-focused care principles and mobility support
• Ability to prioritize tasks in a fast-paced clinical environment
