Physical Therapist
Location: Plover, WI
Contract Type: Perm
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day). Scheduled as a 30 hours/week position with no weekend rotation; travel between sites may be required.

Roles & Responsibilities
- Evaluate patients to identify functional limitations and design individualized treatment plans
- Deliver skilled therapeutic interventions to improve mobility, strength, and independence
- Document patient progress clearly and timely in the medical record
- Collaborate with nurses, therapists, and care teams to coordinate comprehensive care
- Educate patients and families on home programs and safety strategies to support long-term outcomes
- Monitor and maintain therapy equipment and ensure a safe treatment environment
- Contribute to quality initiatives and help improve patient experience and clinical outcomes
Qualifications
- Current Physical Therapist license in Wisconsin or eligibility to obtain licensure
- Graduation from an accredited Physical Therapy program (DPT preferred)
- Basic Life Support (BLS) or CPR certification
- Comfort working in a skilled nursing/long-term care environment preferred
- Strong communication and teamwork skills with a patient-centered approach
- Reliable transportation and willingness to travel between assigned sites as required
Education
- Doctor of Physical Therapy (DPT) preferred; Bachelor or Master in Physical Therapy considered
Knowledge & Skills
- Skilled in patient assessment, therapeutic exercise, and mobility training
- Knowledge of documentation best practices and electronic medical records
- Experience with geriatric rehabilitation and fall prevention strategies
- Ability to communicate clearly with patients, families, and interdisciplinary teams
- Good clinical reasoning, time management, and organizational skills
