Roles & Responsibilities

• Assist residents with daily living activities including bathing, dressing, grooming and toileting with dignity and respect
• Provide safe transfers and mobility support to help patients participate in rehab goals
• Monitor and record vital signs and report changes to the nursing team promptly
• Document care accurately and timely in the patient record to support continuity
• Support the rehab team during therapy sessions and follow care plans
• Maintain a clean, safe environment and follow infection prevention protocols
• Communicate compassionately with patients and families about basic care needs
• Collaborate with nurses and therapists to help achieve positive patient outcomes

Qualifications

• Active CNA certification (required)
• CPR/BLS certification preferred or willing to obtain
• Previous experience in long-term care or rehab is a plus
• Comfortable with patient transfers and basic mobility assistance
• Strong communication skills and a patient-centered approach
• Ability to pass facility background checks and health screenings

Education

• High school diploma or equivalent
• Completion of an accredited CNA program

Knowledge & Skills

• Skilled in activities of daily living (ADLs) support
• Comfortable taking and documenting vital signs
• Knowledge of basic infection control and safety practices
• Ability to follow care plans and verbal/written instructions
• Good interpersonal skills for working with patients, families and the care team
• Basic charting and documentation skills
