Roles & Responsibilities
- Assist residents with daily living activities including bathing, dressing, grooming and toileting with dignity and respect
- Provide safe transfers and mobility support to help patients participate in rehab goals
- Monitor and record vital signs and report changes to the nursing team promptly
- Document care accurately and timely in the patient record to support continuity
- Support the rehab team during therapy sessions and follow care plans
- Maintain a clean, safe environment and follow infection prevention protocols
- Communicate compassionately with patients and families about basic care needs
- Collaborate with nurses and therapists to help achieve positive patient outcomes
Qualifications
- Active CNA certification (required)
- CPR/BLS certification preferred or willing to obtain
- Previous experience in long-term care or rehab is a plus
- Comfortable with patient transfers and basic mobility assistance
- Strong communication skills and a patient-centered approach
- Ability to pass facility background checks and health screenings
Education
- High school diploma or equivalent
- Completion of an accredited CNA program
Knowledge & Skills
- Skilled in activities of daily living (ADLs) support
- Comfortable taking and documenting vital signs
- Knowledge of basic infection control and safety practices
- Ability to follow care plans and verbal/written instructions
- Good interpersonal skills for working with patients, families and the care team
- Basic charting and documentation skills
