Roles & Responsibilities

• Provide compassionate direct patient care and support in the rehab setting
• Administer medications and treatments safely and accurately
• Perform timely patient assessments and escalate changes in condition
• Collaborate with nurses, therapists and the interdisciplinary team for coordinated care
• Document care thoroughly and maintain accurate medical records
• Assist patients with activities of daily living and mobility as needed
• Educate patients and families about care plans and discharge instructions

Qualifications

• Active LPN or RN license eligible for practice in New Mexico (LPN preferred given the posting)
• Current BLS (Basic Life Support) certification required or obtainable upon hire
• Comfortable working in a rehab/acute care setting with strong patient-focus
• Excellent communication and teamwork skills
• Flexible per diem availability to cover scheduled 12-hour day shifts

Education

• Graduate of an accredited practical nursing program for LPN
• Or graduate of an accredited nursing program for RN candidates

Knowledge & Skills

• Strong fundamentals in medical-surgical and rehab nursing care
• Medication administration and safe documentation practices
• Patient assessment, prioritization and clinical decision making
• Familiarity with electronic medical record systems and accurate charting
• Infection control, safety protocols and patient mobility support
• Ability to work effectively within an interdisciplinary care team
