Roles & Responsibilities

• Provide compassionate, patient-centered nursing care across the Uptown Health/Rehab area
• Administer medications and treatments safely and accurately according to orders
• Perform timely assessments and document patient status in the chart
• Support rehabilitation goals by assisting with mobility, ADLs, and therapeutic activities
• Collaborate with the interdisciplinary team to coordinate excellent patient care
• Respond to changes in patient condition and escalate concerns appropriately
• Maintain a safe environment and adhere to infection control and facility policies

Qualifications

• Current LPN license OR RN license in New Mexico (must be active and in good standing)
• Basic Life Support (BLS) certification required; ACLS/PALS as applicable or preferred
• Previous experience in rehab, med-surg, or similar inpatient setting is preferred
• Comfort working overnight shifts and independently managing patient needs
• Strong communication and teamwork skills; dependable and patient-focused

Education

• LPN: Practical Nursing diploma or certificate (state-approved program)
• RN: ADN or BSN from an accredited nursing program (if applying as RN)

Knowledge & Skills

• Proficient with patient assessments, vital signs, and medication administration
• Experience supporting rehabilitation patients and assisting with mobility
• Accurate clinical documentation and familiarity with electronic medical records
• Good clinical judgement, prioritization, and the ability to escalate when needed
• Customer-service mindset when working with patients, families, and staff
