Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to adults in the Uptown health/rehab setting
- Administer medications and treatments safely and accurately
- Assess patient status and report changes promptly to the care team
- Collaborate with nurses, therapists, and other staff to coordinate care plans
- Maintain accurate, timely documentation in the patient record
- Support patients and families with education, encouragement, and clear communication
- Participate in unit-based quality improvement and safety initiatives
Qualifications
- Active LPN or RN licensure (current and in good standing)
- Basic Life Support (BLS/CPR) certification preferred or willing to obtain
- Comfortable with medication administration, wound care, and rehab-focused nursing
- Strong communication skills and a collaborative mindset
- Previous long-term care, rehab, or med-surg experience a plus
Education
- LPN diploma or practical nursing certificate; RN candidates with equivalent credentialing also welcome
Knowledge & Skills
- Effective clinical assessment and critical thinking
- Safe medication administration and documentation
- Familiarity with rehab nursing priorities and patient mobility needs
- Clear verbal and written communication with patients and the care team
- Time management and the ability to prioritize in a dynamic setting
