Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents and patients
- Administer medications and treatments accurately and document thoroughly
- Monitor and record vital signs, reporting changes to the care team promptly
- Assist with daily living activities while promoting dignity and independence
- Collaborate with nurses, therapists and facility staff to deliver coordinated care
- Maintain accurate clinical documentation in the facility charting system
Qualifications
- Licensed Practical Nurse (LPN) license in good standing (state licensure required)
- Current CPR/BLS certification
- Previous experience in long-term care, skilled nursing or similar setting preferred but not required
- Strong communication skills and ability to work well on a multidisciplinary team
- Reliable, professional and committed to high standards of patient care
Education
- LPN diploma, certificate or equivalent practical nursing program completion
Knowledge & Skills
- Patient assessment and basic clinical decision-making
- Medication administration and safety practices
- Vital sign monitoring and early recognition of changes in condition
- Infection prevention and safe infection control practices
- Clinical documentation and basic electronic medical record familiarity
- Effective communication with patients, families and care teams
