Roles & Responsibilities

• Provide compassionate direct patient care and assistance with activities of daily living
• Support safe transfers, mobility, and rehabilitation exercises under direction
• Take and record vital signs and observe patients for changes in condition
• Document care accurately and timely to support continuity and quality
• Communicate clearly with nurses, therapists, and the care team to advocate for patients
• Maintain infection control practices and a safe, clean environment
• Bring positive energy and encouragement to patients working toward recovery

Qualifications

• Current Certified Nursing Assistant (CNA) credential — you are proud of it and can show it
• Comfortable working in a rehab/long-term care environment (Uptown Health/Rehab area)
• Compassionate, patient-focused attitude with good communication skills
• Able to pass facility-required background checks and meet onboarding requirements
• Prior CNA experience is a plus, but enthusiastic learners are welcome

Education

• High school diploma or GED preferred
• Completion of an accredited CNA training program required

Knowledge & Skills

• Skilled in assisting with ADLs: bathing, dressing, toileting, feeding
• Basic vital signs monitoring and reporting
• Understanding of infection prevention and safe patient handling
• Good documentation habits and attention to detail
• Teamwork, reliability, and strong interpersonal skills
• Comfort supporting patients in a rehabilitation-focused setting
