Roles & Responsibilities

Conduct thorough clinical assessments, screenings, diagnoses, and treatment planning for patients from birth to elders
Deliver brief therapy and continuing care that truly strengthens families
Lead discharge planning and facilitate patient care conferences for seamless transitions
Coordinate care with hospital teams and community partners for optimal patient outcomes
Provide passionate client advocacy, skill-building, and support for complex needs like chemical dependency and mental health
Maintain precise, compliant documentation to uphold the highest standards of care

Qualifications

Master's Degree in Social Work, Psychology, Counseling or related field
Licensed as LCSW, LPC, or LMFT (or in process)
Valid Alaska Driver's License (obtainable within 6 months)
At least 21 years old
Clinical Competency required within 3 months and every three years

Education

Masters Degree in Social Work, Psychology, Counseling or related discipline from an accredited institution

Knowledge & Skills

Expertise in discharge planning and care coordination
Skilled in interviewing techniques and crisis intervention
Strong case management with tact and independent judgment
Knowledge of ethical practices, patient rights, and documentation regulations
